Community LIFE's Mission

Our Vision


The Community LIFE Program will be a bridge between, and a resource to traditional and new forms of health care, by becoming a model provider of comprehensive health and social services, integrated acute and long-term geriatric care, and coordinated, person-centered services.
 

Community LIFE is a non-profit organization which provides comprehensive health and social services to frail, elderly adults so that they can continue to live in their homes. This model of care is known nationally as PACE, or Program of All-Inclusive Care for the Elderly. PACE was first authorized as a demonstration by Congress in the 1980's to provide an alternative to nursing home living for the elderly. The Balanced Budget Act of 1997 gave PACE programs provider status under Medicare and gave State Medicaid agencies the option to include it as a benefit.

Community LIFE has contracts with Medicare and the Pennsylvania Medical Assistance Program and is also able to accept private payments from individuals who do not qualify for these programs. We are able to use these payments in a non-traditional manner to provide preventative and supportive services to keep elders living independently in the community.

Community LIFE is a non-profit program sponsored by the Jewish Association on Aging,
Presbyterian Senior Care and UPMC. Together these organizations bring expertise and commitment in caring for older adults.
