Our mission is to provide a continuum of client-focused services which enhance quality of life and maximize independence. Through innovative partnerships and the interdisciplinary team we strive to achieve the following for frail, older adults: enhance quality of life, enable autonomy, maximize dignity, enable living in the home and community for as long as is feasible and to preserve and support the family unit. |
