Community LIFE is designed to serve elders who live in the community or may wish to return to the community, if feasible. These elders may reside in their own home or with their families. We seek to begin serving elders before they are in need of 24-hour supervision so that the elder can benefit from early intervention and our extensive preventative services thus, allowing them to stay in their own homes for as long as feasible and enjoy the highest quality of life possible. |

- At least 55 years of age
- Have been determined clinically eligible for the program
- Agree to accept all health care from Community LIFE staff and its providers
- Have health-related problems which make it difficult to manage their own needs independently at home, and qualify them for nursing home level care
- Ability to live safely at home and community with some assistance
