Eligibility
Eligibility Requirements
- Are at least 55 years of age
- Have been determined clinically eligible for the program
- Agree to accept all health care from Community LIFE staff and its providers
- Have health-related problems which make it difficult to manage their own needs independently at home, and qualify them for nursing home level care
- Ability to live safely at home and community with some assistance
- Live in one of the following zipcodes:

Application Process
Contact the Intake department at 1-866-419-1693. An Enrollment Coordinator will explain the program and answer
any questions you may have. He/she will assist you in the application process.
