Why should I refer my client to Community LIFE?

Community LIFE is able to provide comprehensive care to seniors in the community. We are able to provide services that traditionally are not paid for by Medicare and Medicaid.

These services include in-home services such as personal care, house keeping and meal preparation as well as transportation, adult day services and home modifications.

If my client joins Community LIFE, do they need to attend the adult day health center?

No. The interdisciplinary team along with input from the participant and family determine the need for Day Center attendance.

Who is an appropriate referral for Community LIFE?

Community LIFE services seniors 55 and older who meet Nursing Facility Level of Care. Appropriate referrals may be clients who:

Have frequent falls
Miss medical appointments
Have difficulty with medication management
Have a history of multiple emergency room visits, hospital stays or short term nursing home stays
Be socially isolated

What happens if my client joins Community LIFE but doesn't like it?

Community LIFE is a program of choice. If a participant is unhappy and wants to disenroll, our Social Worker will assist them with the disenrollment process, including getting the participant set up with former providers.
