Job Title: Van Driver
Department: Transportation
Schedule: Determined by transportation schedule
Summary of Job Duties:
The Van Driver is primarily responsible for transporting program participants to and from the day center and their residence, to outside appointments and activities, delivering DME, etc as requested by the Transportation Supervisor.

Required Education:
High School Diploma or equivalent.

Required Experience:
Previous transportation, DME and/or supply experience including experience transporting individuals with special mobility needs or equivalent. Prior experience in the geriatric field helpful.

Required Skills:

  • Knowledge of the area roads in the programs catchment area.
  • Ability to read a road map.
  • Ability to deal professionally and diplomatically with participants and other staff.
  • Good interpersonal skills
  • Be mentally alert and have the ability to effectively plan and organize assignments in the most efficient manner.
  • Must be flexible, adaptable and able to function in stressful situations.
Required Certifications/Licensure:
Valid PA driver’s license with a two-year history of no traffic violations. Ability to obtain a public passenger’s license.

Job Title: Program Assistant/Home Health Aides (Full-Time)
Department: Day Center/Home Care
FLSA Status: Non-Exempt
Summary of Job Duties:
The Program Assistant/Home Health Aide is responsible for providing assistance to frail elderly participants with activities of daily living, recreational activities and performing personal care services as needed in the Day Center and in their place of residence. Overtime may be required on an as needed basis to fill homecare assignments.

Required Education:
High School Diploma or equivalent.

Required Experience:
One or more years of experience working with a frail elderly population is required. Experience as a personal care aide, home health aide or nursing assistant is desirable.

Required Skills:

  • Discretion, tact, patience, and good judgment.
  • Empathetic and compassionate attitude in caring for the elderly.
  • Ability to understand and carry out non-technical verbal and written directions, record messages, and keep records.

Job Title: Enrollment Assistant
Department: Marketing
Schedule: Monday-Friday 8:00 a.m. - 4:30 p.m. or as needed; local travel required. Occasional evenings and weekends may be required.
Summary of Job Duties:
The Administrative Assistant–Marketing provides administrative support to the department in a variety of capacities including database management, telephone coverage, copier projects, mail handling, supplies control, processing of letters/correspondence, project management, generating reports, invoice/expense reporting, and assisting with the completion of assigned projects.

Required Education & Experience:
A High School Diploma or equivalent and at least 2 years of administrative assistant experience required. Business School, Associate’s degree or equivalent is a plus. Marketing experience is preferred.

Job Title: RN Home Care Coordinator (Full-Time)
Department: Nursing
Schedule: 7:30 a.m. – 4:00 p.m. with flexibility to be on-call. Occasional overtime may be necessary.
Summary of Job Duties:
The RN Team Leader/Home Care Coordinator oversees the daily operation of the primary care clinic and home care at the assigned Community LIFE Center. Functions supervised include but are not limited to: nursing care; medication management; compliance; safety; patient scheduling; maintenance of proper medical records; supply management; and reporting and monitoring of clinical/quality data. The RN Team Leader/Home Care Coordinator coordinates clinic services with the Primary Care Physician, Interdisciplinary Team (IDT), home care scheduler, pharmacy, laboratory, or other providers/vendors involved in the delivery of primary care/outpatient services to LIFE enrollees. As a member of the Interdisciplinary team the RN Team Leader/Home Care Coordinator also assists with the development of policies and procedures, strategic and quality plans as assigned. The RN Team Leader/Home Care Coordinator coordinates quality audits and process improvements within the clinic also provides the technical, organizational and communication skills necessary to efficiently assess the need for, and coordinate the delivery of, home care and clinic services. This person must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and Community LIFE standards.

Required Education & Experience:
Must be a graduate of an accredited nursing school with at least one year of experience as a Registered Nurse working with a frail, elderly population preferably in home health care or community health. One to two years of clinical management experience plus teaching patients and other health care workers preferred.

Required Certifications and Licensure:
Licensed Registered Nurse in Pennsylvania

Required Skills:

  • Knowledge of the medical, social, and emotional needs of a frail, elderly population
  • Effective written and oral communication skills
  • Strong organizational and planning skills; ability to manage multiple priorities
  • Must be able to work independently and to utilize critical decision making skills
  • Working knowledge of utilization review, quality assurance and managed health care concepts

Job Title: Service Coordinator
Department: Marketing
Schedule: Monday-Friday 8:00 a.m. - 4:30 p.m. or as needed; extensive local travel required
Summary of Job Duties:
Provides coordination of services and outreach to seniors in the Community LIFE service area. This individual will establish links and build relationships with agencies and service providers that will help enhance the quality of life for the residents. Additionally, this person will serve as a liaison when necessary between the Senior Hi-Rise Property Management team and the resident to offer referral support.

Required Education & Experience:
Must be a graduate of an accredited nursing school with at least one year of experience as a Registered Nurse working with a frail elderly population preferably in home health care or community health. One to two years of clinical management experience plus teaching patients and other health care workers preferred.

Requirements:
BA in Human Services, plus 1 year of working with the frail or elderly; Experience with community organizations, social service systems and computer literacy.

Knowledge, Skills, and/or Abilities:

  • Ability to work effectively with culturally, economically and educationally diverse populations
  • Ability to form positive interpersonal relations in dealing with a wide range of staff as well as external contacts
  • Self-starter with strong time management skills
  • Work experience in healthcare and/or aging services highly desirable
  • Knowledge of catchments’ area communities (primarily East End)
  • Working knowledge of local health care and geriatric service networks
  • Interest in geriatrics and community-based programming and the ability to appreciate and enjoy working with elderly individuals

Job Title: Associate Chaplain
FLSA Status: Exempt
Department: Social Services
Schedule: 7:30 a.m. – 4:00 p.m. or as needed
Summary of Job Duties:
The Chaplain is responsible for the spiritual counseling, non-judgmental listening, direction, guidance, and support for all Community LIFE (CL) participants. The Chaplain must demonstrate the knowledge and skills necessary to assess, plan care for and provide spiritual services to frail elders according to assigned responsibilities and CL standards. Qualifications:

Master’s degree in Divinity and one year experience as a Chaplain/Spiritual Counselor providing services to a frail or elderly and/or mentally and physically handicapped population preferably in a health care setting or an equivalent combination of education and experience. Ordination preferred. The ability to work independently as well as to work cooperatively with others and communicate effectively is required. Strong organizational and planning skills and the ability to manage multiple priorities are also required. The ability to use appropriate interpersonal styles and methods to reduce tension or conflict between two or more people is also a must.


Job Title: Russian Language Interpreter (Part-Time)
Department: Day Center
Schedule: 8:00 a.m. – 2:00 p.m. ( Monday, Tuesday, Wednesday)
Summary of Job Duties:
The Russian Language Interpreter facilitates communication by providing professional interpretation and translation services to our participants in the day center, in the home and during medical appointments.

Required Education & Experience:
High school diploma and/or equivalent. Must have through knowledge of and fluency in Russian and English Languages. Must have experience providing interpretation and translation services. Must have knowledge of medical terminology.

Required Skills:
BA in Human Services, plus 1 year of working with the frail or elderly; Experience with community organizations, social service systems and computer literacy.

  • Ability to listen to and understand information and ideas presented through spoken words and sentences
