Community LIFE provides older adults with worry-free, fully integrated and coordinated health and social services, usually at no cost to qualified individuals.

Qualifying participants in the Community LIFE program are transported to our day health center on an as-needed basis, to receive healthcare and social services, meals and to participate in various activities. Additional services are also available for participants in their homes.
