Meet Your Team

The specialists that specialize in you.

Nurses

Our team of nurses is able to provide breathing treatments, IV medication, wound care, vaccinations, home visits and medication counseling and management. There’s always a nurse on call 24 hours a day, 7 days per week, 365 days per year.

Therapists

Certified Physical and Occupational therpaists meet with you to design a care plan that can include skilled therapy services. They will help you with building strength, after an injury and with maintaining your level of independence. You will also have access to wellness programming and services.

Dieticians

Our registered Dieticians will meet with you to discuss your individual dietary needs. They can recommend a menu that includes supplements and that can meet the needs of folks with diabetes, heart conditions, and more.

Social Workers

The social workers are advocates for you. They can assist with applications for benefits and can coordinate additional resources to meet all of your needs; in the center and in the community.

Providers

As a member of the interdisciplinary team, our physicians get to know you on a personal level. They manage your medical care and help you make decisions to stay healthy and independent in the community.

Transportation

Our transportation will take you to and from the center each day and to all of your outside medical appointments. Our Chair-to-chair service means our drivers will meet you in your home, help you with your coat, and wait for you until your appointment is over.

I was going to a different community service, but I didn’t like it. We would have lunch but the rest of the day we would just sit around doing nothing. One day, I saw a pamphlet about Community LIFE, and I went from doing nothing to having friends and being able to laugh and joke around. My family, they are really glad that I finally got something to do, and I am not just sitting at home.

Your Medical Team

At Community LIFE, your doctor is on-site – no appointments are necessary. Plus, if needed, we’ll coordinate outside specialist appointments and provide transportation. Our ability to coordinate care ensures that participants receive all the information necessary to make informed, timely medical decisions.

Where We Come From

2021

Milestone

Community LIFE opens PACE site in Somerset, PA, to serve the southern two-thirds of the county.

2020

Milestone

Community LIFE opens PACE site in Rostraver Township to better meet the needs of the Mon Valley.

2018

Milestone

CMS approves expansion of Logans Ferry to a full PACE site. Community LIFE opens Bedford County Site after being awarded the service area by the Office of Long-Term Living. Community LIFE is also awarded two-thirds of Somerset County with plans to open after Bedford reaches capacity.

2017

Milestone

With enrollment exceeding capacity at the Tarentum Center, Community LIFE opens a licensed adult daily living facility as an alternate care setting in Lower Burrell. Community LIFE moves from its 13,000 square foot site in McKeesport to a newly renovated 30,000 square foot state-of-the-art site at Olympia Shopping Center.

2016

Milestone

Community LIFE awarded 13 additional zip codes in the “Mon Valley” – the Charleroi/Monessen/Donora area in Washington and Westmoreland Counties – areas initially served by the McKeesport site.

2014

Milestone

Community LIFE approved by CMS to serve nine additional zip codes in northern Westmoreland County including Apollo, Leechburg, Vandergrift, and Murrysville – locations initially served by the Tarentum site.

2007

Milestone

The Tarentum Center opens, serving Tarentum (Allegheny County) and New Kensington (Westmoreland) areas.

2004

Milestone

Community LIFE becomes permanent provider under the Program of All-inclusive Care for the Elderly (PACE). CMS approves expansion to East Liberty through contract with Vintage Adult Day Services.

2003

Milestone

Community LIFE program assigned three Westmoreland County zip codes.

2002

Milestone

The Homestead LIFE center opens with 22 participants transferring from the McKeesport Center and three additional enrollees. Census nearly triples in one year. Community LIFE applies for and is assigned 22 additional zip codes in Allegheny County, as well as the entire counties of Washington, Fayette and Greene by the PA Department of Welfare.

2000

Milestone

The new LIFE center opens in McKeesport. Projected enrollment exceeds expectations by 50 percent in first year.

1994

Milestone

Pittsburgh Care Partnership established as a non-profit to support health and human services to the vulnerable elderly and chronically ill.
