
The Interdisciplinary Team or IDT is the cornerstone of Care Management for the Community LIFE participant. The IDT assesses need, provides care, coordinates contracted services and monitors each participant's situation. Participants, their family and caregivers along with the IDT create an individualized plan of care. This plan is a holistic approach to managing care in the community.
Community LIFE's IDT consists of geriatric physicians, RNs, social workers, therapists, dieticians, pharmacists, personal care aides, van drivers and others. Each plan of care developed by this team is tailored to meet the unique needs of each participant. The program focuses on preventative care, maintenance of a participant's highest level of functional ability and the early recognition and treatment of symptoms that can signal major problems.
Our scope of services spans preventative care, acute care and chronic care, all the way into long-term care. There are no gaps in the system. All the needs of the individual are met through our integrated, comprehensive approach.
